LOST YOUR CARD?

There is a $5 charge for the replacement of lost of misplaced cards.

Card holders should report the loss or theft of cards immediately to guard

against fraudulent use. This can be done in several ways. Users can disable

cards at any time by logging on to the Online Card Office. Users without

online access should call the BENGAL-ID office at (337) 550-1307 to report

a card missing. After hours users should leave a voicemail with the following

information: (Name, student ID number, social security number, date of birth,

telephone number at which you can be contacted and and the last known date

and time the card was in their possession.)
